We thank you for trusting in Kid Kare Pediatrics. For our new patients, to save the trouble and time, please follow the following instructions. ​

  1. ​Prior to arriving to Kid Kare Pediatrics, please contact your Medical Insurance provider and verify that Dr. El-Takech is within your insurance network. Thereafter, ask the operator to list Dr. El-Takech and Kid Kare Pediatrics as your primary care physician and to list Dr. El-Takech on your physical insurance card on your insurance card. As a method of improving delayed time, provide the operator with Dr. El-Takechs' NPI# (1841240538), so the operator can find Dr. El-Takech in their system in a more efficient manner.

  2. Please download and complete the New Patient Registration Form & Health Questionnaire Form prior to arriving to Kid Kare Pediatrics. 

  3. If you are not accompanying your child, and are entrusting someone else to bring your child in, please download and complete the "Third-Party Consent Form.
